Advance Care Planning Workshops

Have you had your advance care planning conversation but need help with the next step? Want to get started on your advance care plans but stumped about how to start? National Healthcare Decisions Day is April 16, 2018, and Interfaith Community Services (ICS) is celebrating by offering THREE opportunities, on April 16, 17 and 18, when you can participate in their highly motivating and informative Advance Care Planning workshop!

Meet & Complete!

Advance care planning is a way to help insure that your values in life and wishes for care will be honored all the way through your life, if you are ever unable to speak for yourself. Meet and Complete is a workshop designed to help people begin, complete or revise their advance care planning documents. Topics include:

  • Durable Power of Attorney for Healthcare – Your choice of a spokesperson to make healthcare decisions for you if you become incapacitated.
  • Living Will – Your decisions about the types of healthcare treatments you want or do not want as you approach the end of life.
  • “Do Not Resuscitate” – Optional: Your declaration that you do not wish to be resuscitated if your heart or breathing stops.

Light snacks available
Raffle prizes for those who complete documents
